CHICAGO – State Rep. Jaime Andrade, D-Chicago, and state Sen. Iris Martinez, D-Chicago, will host a Family Wellness & Back to School Fair, offering free school supplies and health services for local families.
“The health and well-being of the entire family is critical for a child’s success in school,” Andrade said. “With the demand for medical care constantly rising, I want to bring free health care services to as many people as possible. I also know that it can be difficult for working parents to navigate the different government services out there so I will also have representatives from various agencies at the city, county and state level to provide information and guidance on how to apply for services like Medicaid.”
Andrade’s event is free and open to the public. Free backpacks and school supplies will be provided for kids returning to school. Health services for children include back to school physicals, immunizations (must bring child’s records), All Kids health insurance registration (please bring one pay stub), dental and vision screenings and children’s identification cards. For adults, there will be registration for the City of Chicago CityKey Program, blood pressure screenings, glucose screenings, cholesterol screenings, CountyCare health insurance registration, Family Care and Moms and Babies health insurance registration, Secretary of State Services and information about agencies and services.
